South Dakota is committed to improving access to long-term care at home. Beginning in 2019, Structured Family Caregiving (SFC) is now available to Medicaid eligible families.

What is Structured Family Caregiving?

SFC is a caregiver support program designed for family and friends who care for a loved one at home. Seniorlink’s SFC program provides care with:

  • A tax-free financial stipend
  • Ongoing support from a professional care coach
  • Access to a secure mobile app to connect you with your coach

How do I know if my family is eligible?

Individuals who are eligible for the HOPE Waiver have access to Structured Family Caregiving. Caregivers must live in the same home as those they help, you may qualify!
