South Dakota is committed to improving access to long-term care at home. Beginning in 2019, Structured Family Caregiving is now available to eligible families. With Structured Family Caregiving, eligible families now have financial and professional support available to them to make in-home care possible. 


Structured Family Caregiving (SFC) is a caregiver support program designed for family and friends who care for a loved one at home. If this describes you, SFC through Seniorlink might be the program for you.

This program offers you:

  • A financial stipend
  • Ongoing support from a professional care coach
  • Access to a secure mobile app to connect you with your coach

You know how to be a family member or a friend, but being a caregiver is not always easy. Sometimes it feels like you need formal health care training.  We know that.  With the support of a caregiver coach, you can provide great care at home.

Our coaches are experienced in working with family caregivers. Coaches can talk to you about your loved one’s medical conditions, needs, and any other challenges you might have (for example, managing medications). Our coaches will help give you confidence as a caregiver.  

Caregiving can feel lonely, sometimes but you don’t have to do it alone. We will make sure you have a plan to take care of yourself while caregiving.

There is a financial cost to caregiving. We will make sure you receive the financial stipend, made available by the State to caregivers who choose this program, on time.

Individuals want to stay at home and family caregivers want that too. We believe, with information and support, you can be the kind of caregiver you want and need to be.
